First Visit
After we find a convenient time to meet either in person or virtually then you will complete the 360 Health Assessment Form.

At your appointment, we will go over your goals and your barriers. In addition, we will look over what you eat, your eating behaviors, exercise habits, sleeping and bathroom issues.

Together we will design a personalized
plan for success.
Throughout your appointment, together we will come up with a plan and tolls ll give you reach your goals.
Follow Up Visits
Together we will decide how frequently to meet to help you stay accountable and help you obtain your goals.
In the follow up sessions, we will discuss the barriers that have come about since you set your new plan in place. We will also continue the journey of lifestyle changes. I believe baby steps make the biggest difference.​

In person or virtual
In person is at the office of A Stronger Tomorrow.
Virtual meeting, telehealth, is a meeting via a video call. The video call can be scheduled for when it is most convenient for you and where you have an internet connection. Video calls are made through Simple Practice, a HIPPA compliant program.

If your insurance company does not cover your visits, you will be responsible for the charges.

If I am out of network with your insurance company, I will be happy to provide a receipt that you can submit to your health insurance company for possible reimbursement.
